Guest Artist Contracts

Any guest artist coming to Yale to perform or to conduct a master class must sign an agreement covering their services.  The following forms may be used for either performances or master classes.

The Yale University Standard Performance Contract should be used when payment is coming from a Yale office.  The steps to follow to use this form are:

  1. Download the Yale University Standard Performance Contract and fill out the information to the best of your ability. Questions about this form may be directed to your UP representative or to the Yale Procurement office.
  2. Once it is signed by the guest artist, send the Yale University Standard Performance Contract to purchasing.contracts@yale.edu for the Yale signature.  Only authorized Yale staff may sign on behalf of Yale University.
  3. Send a copy of the completely signed agreement to your guest artist and to your UP representative.

When payment is coming from a Student Organization, the steps to follow are:

  1. Download the Yale University Standard Performance Contract - Student Organization and fill out the information to the best of your ability. Questions about this form may be directed to your UP representative.
  2. Have this form signed by the guest artist and a representative of the student organization.
  3. Send a copy of the completely signed agreement to your guest artist and to your UP representative.

Either of these Guest Artist Contracts should be accompanied by an Assumption of Risk form.
